2011 Troy the Activity Truck Consumer Return Form

1. Please scroll down the page to view your replacement options. Once you have determined your replacement choice, please complete the form and click on the "Process My Request" Button below. Your information will be downloaded to our database and you will automatically be assigned a Transaction ID number.
2. Once you click on the "Process My Request" button a printable version of the Return Form and a link to the prepaid FedEx mailing label will pop up. *Please note that you will need to print or save the Fed Ex label to your computer after filling out the form. If you are unable to print or save the label at this time, please come back and complete the form at a later time when you are able to do so.
   1. Please print the prepaid mailing label.
   2. Please print the Consumer Return Form and be sure to return it with the recalled item.
3. A copy of your Return Form will automatically be sent to the email address you provide. In the event that you do not have access to a printer at this time, you can access and print this information at a time that is more convenient for you.
